You make no mention of age or any other background info, but the simplest case is that of hemorrhoids. Colon cancer, ulcerative colitis, Crohn's disease are other differential diagnoses. In all cases, I assume that he's seen a doctor already, as there is no substitute for taking a look in this situation.
